ELIGIBILITY REQUIREMENTS
- Must have limited income with no Medicaid, Medicare or private insurance, or have a high deductible
- Must be at least 45 years of age (or 40 if you are African American or have had a father, brother or son diagnosed with prostate cancer)
- Must not have previously had prostate cancer
